Here are some key points to keep in mind. Along with completing the application, you must submit a fee of $110.00 payable to “DOPL.” Cash and debit cards are accepted if you apply in person. You must have completed a course in emergency office procedure, which includes a CPR course.
Other requirements in Utah include sending an official transcript directly to the Board from a dental school accredited by the Commission on Dental Accreditation of the American Dental Association that shows the date that you graduated, as well as the degree earned. If you anticipate administering anesthesia, you can also submit an application to administer general anesthesia.
To be eligible to sit for examinations that are mandatory for Utah state licensure, submit an original score report of your National Board Examination scores and the North East Regional Board (NERB) examination scores. Also acceptable are original test reports from the Central Regional Dental Testing Service, Southern Regional Testing Agency or Western Regional Examining Board.
