Becoming a Dentist in Virginia - VA
Most new graduates apply for dental licensure by examination and the Virginia Department of Health Professions (DHP) and the Virginia Board of Dentistry have specific requirements for those who wish to become dentists.
For Virginia dental license applicants applying for license by examination, an application can be downloaded at http://www.dhp.state.va.us/dentistry/dentistry_forms.htm. A passport photo that shows the applicant’s full face, and is not more than six months old, must be attached to the application. A check, cashier’s check or money order made payable to the Treasurer of Virginia in the amount of $400 must be included with the application and sent to:

Virginia applicants must provide an original certification of graduation from their dental school, which must be accredited by the Commission on Dental Accreditation of the American Dental Association. The final original transcript must have a raised SEAL indicating the date the degree was received and the registrar’s signature.
An original score card or report that certifies a passing score of a clinical examination involving live patients is required, as well as scores from results of SRTA, CRDTS, WREB or NERB examinations.
