Missouri Dental Hygienist License

To Obtain a License by Credentials, an applicant must:
  1. Take and pass the Missouri jurisprudence exam.
  2. Pay $155 application fee by check or money order.
  3. Attach a recent photograph.
  4. Provide a final transcript from the appropriate training program showing successful completion.
  5. Make sure national board exam results are sent directly to the state board from the testing agency.
  6. Make sure clinical exam results are sent directly to the state board from the testing agency.
  7. Submit proof that licensure in another state was for at least two years prior to application.
  8. Submit copy of license or certification from the previous state.
  9. Submit proof of current certification in CPR.
