To Obtain a License by Credentials, an applicant must:
- Take and pass the Missouri jurisprudence exam.
- Pay $155 application fee by check or money order.
- Attach a recent photograph.
- Provide a final transcript from the appropriate training program showing successful completion.
- Make sure national board exam results are sent directly to the state board from the testing agency.
- Make sure clinical exam results are sent directly to the state board from the testing agency.
- Submit proof that licensure in another state was for at least two years prior to application.
- Submit copy of license or certification from the previous state.
- Submit proof of current certification in CPR.
