How to Become a Licensed Practical Nurse in Idaho


After successfully graduating from an approved online or campus-based Idaho LPN program, aspiring LPNs in Idaho need to visit the Idaho Board of Nursing website and download an “Application for Nurse Licensure by Examination” form. An application fee of $75 must accompany the submission of the application. This application will first request applicant’s name, address, phone number, place of birth, date of birth, social security number and current photograph.

Another section pertains to LPN educational program information such as program name, location, dates attended, and type of degree or certificate earned. Furthermore, this application will require applicants to complete a census questionnaire along with any admittance regarding misdemeanor and felony convictions, past disciplinary actions in nursing, physical and mental conditions, and drug and alcohol issues that may negatively impact applicant’s ability to practice nursing. Another section will address the applicant’s employment status and details thereof. In addition, applicant will need to complete a fingerprint card ($30) with the application, which is used to perform a criminal background check.

The licensing application will also be used to judge the eligibility of an applicant’s qualifications to take the national nursing competency examination, called the NCLEX-PN. Therefore, applicants must obtain an “Affidavit of Graduation” form from the IBN website. This form is to be completed by a director or administrator from the LPN program attended. The completed form must be directly sent to the Idaho Board of Nursing to verify successful completion and graduation. After an applicant is found to be eligible for NCLEX-PN examination, an “authorization to test” letter will be send to the applicant. Applicants can then contact Pearson Vue to register for examination and schedule testing. The cost for examination is $200.
