How to Become a Licensed Practical Nurse (LPN) in Indiana
After successfully completing an online or campus based Indiana LPN program accredited by either the National League for Nursing Accrediting Commission (NLNAC), or the Commission on Collegiate Nursing Education (CCNE), individuals will need to visit the Indiana Board of Nursing and obtain an “Application for Licensure by Examination for Graduates of U.S. Nursing Schools” form. This form will request the following:
- Applicant’s personal information such as legal name, address, phone number, date of birth, place of birth, social security number, and e-mail address
- Applicant’s formal nursing education details including name, location, date of enrollment, date of graduation, and type (PN program, associate, baccalaureate, diploma)
- Applicant’s high school education information or copy of GED scores
- If applicant is currently licensed or was formerly licensed to practice a health care profession in another state(s), applicant will need to provide license type, location, license number, date of issue and status information
- Applicant must admit past professional disciplinary actions, license denials, illegal drug use, misdemeanor and/or felony convictions, alcohol abuse, and malpractice involvements that may influence the Board’s decision to approve licensure to practice in Indiana
- An application processing fee of $50
- Current photograph of applicant
- Application will direct applicants to download a “Certificate of Completion” form to be filled out by the director/administrator of the LPN program where education was received and sent directly to the Indiana Board of Nursing.
- Applicants must register to take the LPN national competency examination, called the NCLEX-PN. To register, applicants must contact the National Council of State Boards of Nursing. The examination fee is $200. For more information call (866) 496-2539.
Once the license application is approved by the Indiana State Board of Nursing, and the NCLEX-PN is successfully passed, a license to practice in Indiana will be granted. All LPN licenses expire on October 31st of all even-numbered years, at which time a renewal application and $50 renewal fee will be required.
